Terms of Service
Last updated October 3, 2026
These terms are an agreement between you and FormPilot for using the FormPilot website and service. By creating an account or using FormPilot you agree to them. Contact: support email coming soon.
What FormPilot is
FormPilot helps you fill in official California DMV forms and print them onto the official paper form. FormPilot is an independent service and is not affiliated with, endorsed by, or part of the California Department of Motor Vehicles.
You are responsible for the information you enter, for checking every printed form before you sign it in ink, and for submitting it correctly. We do not guarantee that the DMV will accept a form. Printer alignment depends on your printer; use the alignment tools and check a test print.
Your account
- You must be 18 or older and give accurate information.
- An account is for one person. Plans are priced per user, and an account can be signed in on one device at a time.
- Keep your sign-in details safe. You are responsible for activity on your account.
Free printed forms and plans
- Every person may print 10 forms for free. A printed form is one vehicle printed for the first time on the buying or selling side; reprints of the same vehicle are free.
- The free forms are per person, not per email address: accounts that share a device, network or email inbox share the same free forms.
- Paid plans are per user and renew or end as agreed when the plan is activated. Plan limits and prices are shown on the Pricing page and may change; changes don’t affect a period you have already paid for.
- Until online payment is available, plans are activated by us after you request one, and we send payment details.
- Promo codes are optional discounts, valid only during their dates, once per customer, and may be ended at any time. They have no cash value.
Acceptable use
You agree not to:
- use FormPilot for fraud, to create false documents, or for anything unlawful;
- enter another person’s information without the right to use it for the transaction;
- get around printing limits, for example by creating extra accounts or changing technical settings;
- copy, resell, reverse-engineer, overload or access the service by automated means without permission.
Your information
You keep ownership of the information you enter. You allow us to store and process it to provide the service, as described in our Privacy Policy.
Availability and changes
We work to keep FormPilot available and accurate but provide it “as is”, without warranties to the extent the law allows. We may improve, change or discontinue features. We may suspend accounts that break these terms. You can delete your account at any time from the Account page.
Limitation of liability
To the extent the law allows, FormPilot is not liable for indirect or consequential losses, or for losses caused by information entered incorrectly, printing misalignment, or a form not being accepted. Our total liability for any claim is limited to the amount you paid us in the three months before the claim.
Law
These terms are governed by the laws of the State of California.
Changes to these terms
We may update these terms. We will change the date above and, for important changes, tell you in the app. If you keep using FormPilot after a change, the new terms apply.
